In my experience, if there's roof above the stain, it's most likely a roof plumbing vent boot. The soft pliable rubber that seals around the pipe becomes brittle over time and no longer forms a good seal.

Or, if there's a second floor above the stain, it's most often a leaking tub or shower drain. Specifically where the drain pipe connects to the tub or pan. It's almost always a plumbers putty or rubber gasket failure.
